Abstract

ABSTRAK PT. Arfindo Bersinar merupakan perusahaan yang bergerak dibidang jasa kalibrasi. Pelayanan yang diberikan berupa jasa kalibrasi dan produk sertifikat kalibrasi. Sertifikat kalibrasi merupakan produk yang penting bagi pelanggan yang menerapkan sertifikasi secara nasional dan internasional dalam hal jaminan mutu untuk kepuasan pelanggan. Oleh karena itu, kualitas dari sertifikat kalibrasi yang dihasilkan penyedia layanan kalibrasi sangat perlu diperhatikan. Tanpa hal tersebut maka pelanggan bisa beralih ke perusahaan kalibrasi yang lain. Kualitas sertifikat kalibrasi yang dihasilkan oleh PT. Arfindo Bersinar tidak semua berkualitas baik. Pada bulan Januari-Desember 2019 terdapat 71 cacat dari 521 sertifikat atau berkisar 13.63% cacat melebihi batas standar dan menjadi temuan audit eksternal sehingga perlu perbaikan terhadap kondisi tersebut. Metode FMEA dan FTA untuk kombinasi mencari sumber masalah dan prioritas perbaikan masalah serta peta kendali p untuk kontrol hasil perbaikannya. Hasil dari penelitian didapatkan bahwa: 1) Rumus pada perhitungan ketidakpastian tidak berfungsi dengan baik ditandai dengan nilai RPN 400, namun setelah dilakukan perbaikan nilai RPN berkurang menjadi 216; 2) Sumber data dan tujuan link data salah penempatan dengan nilai RPN 378, namun turun menjadi 200 setelah perbaikan; 3) Menyalin ulang master perhitungan dengan nilai RPN 350, dan menjadi 180 setelah dilakukan perbaikan. Beberapa tindakan perbaikan yang dilakukan adalah dengan mengevaluasi dan memperbaiki rumus perhitungan, membuat rumus otomatis hubungan link data, serta sosialisasi ulang pengisian tanggal kalibrasi. Rata-rata kualitas sertifikat kalibrasi timbangan elektronik sesudah perbaikan sebesar 94.92% dan peningkatan kualitas yang dihasilkan sebesar 7.80%. The services provided are in the form of calibration services and calibration certificate products. Calibration certificate is an important product for customers who apply national and international certification in quality assurance for customer satisfaction. Therefore, the quality of the calibration certificate produced by the calibration service provider needs to be considered. Without this, the customer can switch to another calibration company. The quality of the calibration certificate produced by PT. Arfindo Bersinar is not all of good quality. In January-December 2019, there were 71 defects from 521 certificates or around 13.63% defects that exceeded the standard limit and became external audit findings so that this condition needs to be corrected. FMEA and FTA methods for a combination of finding the source of the problem and the priority of fixing the problem and the control chart p to control the results of the repair. The results of the study found that: 1) The formula for calculating uncertainty does not function properly marked with an RPN value of 400, however after corrections the RPN value is reduced to 216; 2) The data source and destination of the data link were misplaced with an RPN value of 378, but dropped to 200 after correction; 3) Re-copy the master calculation with an RPN value of 350, and it becomes 180 after repairs are made. Some of the corrective actions taken were evaluating and improving the calculation formulas, creating automatic formulas for data link relationships, and socializing the filling of calibration dates. The average quality of the electronic scale calibration certificate after repair is 94.92% and the resulting increase in quality is 7.80%. Keywords: FMEA, FTA, RPN, Defect, Quality, Certificate
