Abstract
Industri ban mobil memberikan perhatian penuh terhadap implementasi dari standar International Automotive Task Force (IATF) 16949:2016 menuntut lebih detail terhadap perbaikan variasi, defect, waste dan kegagalan produksi yang akan menjadi sebuah indikator kinerja perusahaan dalam perbaikan mutu. Tujuan dari penelitian ini yaitu mengetahui faktor-faktor penyebab terjadinya kegagalan produk di proses mixing, mengetahui upaya-upaya yang dilakukan untuk pengendalian dan membandingkan hasil pengukuran produktivitas sebelum dan setelah dilakukan perbaikan. Kondisi pada saat meneliti, ada beberapa masalah pada proses mixing yang memiliki rasio produk NG (Not Good) 2.88%, rasio tersebut melebihi target perusahaan yaitu 1.0% dari total produksi. Untuk menganalisis semua potensi yang menyebabkan produk NG pada proses mixing menggunakan metode Failure Mode and Effect Analysis (FMEA) dan Quality Assurance Network (QA Net) untuk mengidentifikasi mode potensi kegagalan, potensi penyebab kegagalan, pencegahan dan deteksi. Dengan menggunakan metode tersebut, rasio defect turun menjadi 0.50% yang menunjukkan keefektifan dalam melakukan perbaikan.
