Abstract

PT Santoso Teknindo, also known as STI, is a company engaged in manufacturing for the automotive, medical, cutting tools and sheet metal industries. The phenomenon of production quantity that occurred during July - December 2019 PT Santoso Teknindo is still a high production defect rate, compared to the standard defect max of 2.5%. With this phenomenon, this study aims to determine the highest defect type and provide suggestions for improvement with the method used, namely DMAIC (Define, Measure, Analyze, Improve, Control) by calculating the value of production sigma which will be compared with the standard sigma value of PT Santoso Teknindo's factory , as well as using the FMEA (Failure Mode and Effect Analysis) tool used for scoring to determine the main priority causes of defects in order to provide recommendations for improvement. The Define Phase makes for identifying the process flow or SIPOC and determining the description of the CTQ (Critical To Quality). At the Measure stage the results of the calculation of the production sigma obtained DPMO value of 141352.8229 with a sigma value of 2.57 which is then compared to the standard sigma PT STI of 2.78 then there is a gap or difference, followed by the Analyze stage using the Pareto diagram tools and the analysis using the cause diagram consequence. At the imporve stage, the calculation of FMEA obtained the most potential problem causing factors with a RPN (Risk Prority Number) of 294 so that it becomes a priority to be improved. The proposed improvement will be elaborated by an analysis of 5W + 1H at the Improve stage. Finally, a control proposal is given to avoid new problems at this stage of Control. Keywords: DMAIC, six sigma, FMEA, pareto diagram, cause-effect diagram PT Santoso Teknindo atau dikenal dengan STI adalah perusahaan yang bergerak dalam bidang manufaktur untuk industry otomotif, medis, cutting tools dan sheet metal. Fenomena kuantitas produksi yang terjadi selama bulan Juli – Desember 2019 PT Santoso Teknindo masih tingginya angka defect produksi, dibandingkan dengan standar defect maks sebesar 2,5. Dengan fenomena tersebut maka penelitian ini bertujuan untuk mengetahui jenis defect tertinggi dan memberikan usulan perbaikan dengan metode yang digunakan yaitu DMAIC (Define, Measure, Analyze, Improve, Control) dengan menghitung nilai sigma produksi yang akan dibandingkan dengan nilai sigma standar pabrik PT Santoso Teknindo, serta menggunakan tools FMEA (Failure Mode and Effect Analysis) yang digunakan untuk skoring penentuan proritas utama penyebab defect yang guna memberikan rekomendasi usulan perbaikan. Tahap Define membuat untuk mengidentifikasi alur proses atau SIPOC dan menentukan deskripsi dari CTQ (Crtitical To Quality). Pada tahap Measure hasil perhitungan sigma produksi diperoleh nilai DPMO 141352,8229 dengan nilai sigma 2,57 yang selanjutnya dibandingkan dengan sigma standar PT STI sebesar 2,78 maka terdapat gap atau selisih, dilanjutkan tahap Analyze menggunakan tools diagram pareto dan analisanya menggunakan diagram sebab-akibat. Pada tahap Imporve, perhitungan FMEA diperoleh faktor penyebab permasalahan yang paling potensial dengan nilai RPN (Risk Prority Number) sebesar 294 sehingga menjadi prioritas untuk dilakukan perbaikan. Usulan perbaikan akan dijabarkan dengan analisa 5W+1H pada tahap Improve. Terakhir diberikan usulan pengendalian untuk menghindari timbulnya masalah baru pada tahap Control ini.
