Abstract
The fastener company is a multinational company that produces various types of fastener products such as bolts, nuts, rivets, collars, and screws. One of the bolt products, which is a 7/16 NF x 40.2 mm shoulder bolt, is experiencing quality problems due to the detection of defective products that have been received by consumers. So that consumers submit claims which result in the company having to replace products with new products. This study aims to identify the causes of production process failures and provide quality improvement proposals to reduce the failure rate of shoulder bolt products. Shoulder bolt 7/16 NF x 40.2 mm, currently still exceeds the quality target set at 1.8 ppm / month. The problem approach used is using a pareto diagram, then analyzing using the Fault Tree Analysis and Failure Mode Effect and Analysis methods. From the results of data processing using pareto diagrams, the dominant type of defect is black line. Based on the analysis using the FTA and FMEA methods, the priority for improvement in the electroplating process with the largest RPN value of 105 is the concentration of the solution below the standard with the causal factor of not checking the concentration of the solution regularly. While the priority of improvement in the sorting process passes with the largest RPN value of 126, namely the method of shearing is not optimal. The proposed improvement using 5W + 1 H is the concentration of the solution periodically by the QC analyst and replacing the solution every 3 days by considering the results of checking the concentration of the solution so that the consistency of the solution concentration remains and making changes to the ork method by lifthing the product and rotating the product 360° using a magnifier (loop) for the sorting process. Keywords: Customer Claim, Shoulder Bolt, Defect Product, Fault Tree Analysis (FTA), Failure Mode and Effect Analysis (FMEA) Perusahaan fastener merupakan salah satu perusahaan multinasional yang memproduksi berbagai macam jenis produk fastener (pengikat) seperti bolt, nut, rivet, collar, dan screw. Salah satu produk bolt yaitu shoulder bolt 7/16 NF x 40.2 mm mengalami masalah kualitas karena terdeteksinya produk cacat yang sudah diterima oleh konsumen. Sehingga konsumen mengajukan klaim yang mengakibatkan perusahaan harus melakukan penggantian produk dengan produk yang baru. Penelitian ini bertujuan untuk mengidentifikasi penyebab kegagalan proses produksi dan memberikan usulan perbaikan kualitas untuk mengurangi tingkat kegagalan pada produk shoulder bolt. Shoulder bolt 7/16 NF x 40.2 mm, saat ini masih melebihi sasaran mutu yang ditetapkan sebesar 1.8 ppm/bulan. Pendekatan masalah yang digunakan yaitu menggunakan diagram pareto, lalu analisis menggunakan metode Fault Tree Analysis dan Failure Mode Effect and Analysis. Dari hasil pengolahan data menggunakan diagram pareto didapatkan jenis cacat dominan yaitu black line. Berdasarkan analisis menggunakan metode FTA dan FMEA yang menjadi prioritas perbaikan pada proses elektroplating dengan nilai RPN terbesar 105 yaitu konsentrasi larutan dibawah standar dengan faktor penyebab tidak dilakukan pengecekan konsentrasi larutan secara rutin. Sedangkan prioritas perbaikan pada lolos proses sortir dengan nilai RPN terbesar 126 yaitu metode cara geser tidak optimal. Usulan perbaikan menggunakan 5W + 1 H adalah konsentrasi larutan secara berkala oleh QC analis dan mengganti larutan 3 hari sekali dengan mempertimbangkan hasil pengecekan konsentrasi larutan agar konsistensi konsentrasi larutan tetap dan melakukan perubahan metode kerja dengan cara produk diangkat dan memutar produk 360° menggunakan magnifier (loop) untuk proses sortir.
