Abstract
PT XYZ was a technical service company, survey, assessment, audit, and consulting. The aimed of this research was to analyze the failure factors of PT. XYZ 2018 – 2020 in terms of time, cost, labor, Health, Safety, and Environment (HSE), and quality based on the Success Project Factor (SPF). Population 183 projects with Non-Probability Sampling technique. After knowing the category, the researcher analyzed by fishbone and pareto to identify problems. The results of the analysis showed Schedule Performance Index (SPI) < 1 indicated the project in the late category, the Cost Performance Index (CPI) < 1 indicated cost overrun. Safety Performance Index (SFPI) > 0 indicated the project did not get the K3 target, the Client Satisfaction Index (CSI) = 34.03, explained was in the dissatisfied category, then Productivity Coefficient Plan < Realization, it meant the workforce was less productive. After analysis fishbone and pareto the data showed the highest cause was 13% due to lack of supervision, project cost aspects were 13% due to delays, in the HSE project aspect cause 13% that there was no K3 process before work begins, the quality aspect was 17% due to no training, and the labor aspect was 17% due to poor worker discipline. Keywords: Success Project Factor, Schedule Performance, Cost Performance, Safety Performance, Productivity Coefficient, Client Satisfaction PT XYZ merupakan perusahaan jasa teknis, survei, pengkajian, auditing, serta konsultasi. Tujuan penelitian untuk menganalisis faktor kegagalan proyek PT. XYZ Migas dan Sistem Pembangkit 2018 – 2020 dalam aspek waktu, biaya, tenaga kerja, Health, Safety, and Environment (HSE), dan kualitas berdasarkan Success Project Factor (SPF). Populasi 183 proyek dengan teknik Non-Probability Sampling. Setelah mengetahui kategori peneliti menganalisis dengan fishbone dan pareto untuk mengidentifikasi permasalahan. Hasil analisis menunjukan aspek waktu Schedule Performance Index (SPI) < 1 ini menunjukan proyek masuk kategori terlambat, Aspek biaya Cost Performance Index (CPI) < 1 ini menunjukan cost overrun. Aspek HSE dengan Safety Performance Index (SFPI) > 0 ini menunjukan proyek tidak memenuhi target K3, aspek kualitas menggunakan Client Satisfaction Index (CSI) = 34.03, ini menerangkan client berada pada kategori tidak puas, aspek tenaga kerja pada Rencana Koefisien Produktivitas Proyek < Realisasi, Koefisien Proyek ini menunjukan tenaga kerja kurang produktif. Hasil analisis fishbone dan pareto, aspek waktu proyek penyebab tertinggi 13% diakibatkan kurangnya pengawasan, aspek biaya 13% diakibatkan keterlambatan, dalam aspek HSE proyek penyebab tertinggi 13% ini diakibatkan tidak ada proses verifikasi K3 sebelum pekerjaan dimulai, aspek kualitas proyek faktor tertinggi 17% diakibatkan tidak ada pelatihan, dan aspek tenaga kerja proyek 17% diakibatkan kedisiplinan pekerja kurang baik.
