Abstract
The rise of the world information currently a significant increase in each time, so that the need for quick information, right and accurate is needed. Useful to make it easier for each users. In the utilization of information technology produces efficiency covering aspects of information management indicated by the speed and accuracy of the processing time, and thoroughness and the truth of the information produced. And trigger any players organization to make a useful applications to conduct the process of data processing. Because is necessary to support services quickly and accurately be able to follow the development of information in the era of globalization at this time, especially in the field of health services. Rini eka putri clinic is one of the health service facilities that hold public health efforts. In the activities recorded in patients medical record service is not optimal because it is done simply and not yet computerized, namely by using paper media. As a result often mixed and data management is not neat. In which there is information about patients consisting of the identity of patients, examination, the treatment, the act of doctor who another service given to patients. Medical personnel operations that resulted in visible trouble. The programming language used it PHP and MySQL database. Keyword: Medical Record, Information System, Waterfall, PHP, MySQL Berkembangnya dunia informasi saat ini yang signifikan meningkat pada setiap waktunya, sehingga kebutuhan akan informasi yang cepat, tepat serta akurat sangat dibutuhkan. yang berguna untuk memudahkan untuk setiap para penggunanya. Pemanfaatan dalam teknologi informasi menghasilkan efisiensi meliputi aspek pengelolaan informasi yang ditunjukkan oleh kecepatan dan ketepatan waktu pemrosesan, serta ketelitian dan kebenaran informasi yang dihasilkan. Dan memicu setiap para pelaku organisasi untuk membuat suatu aplikasi yang berguna untuk melakukan proses pengolahan data. Karena dipandang perlu untuk menunjang pelayanan yang cepat dan akurat agar mampu mengikuti perkembangan informasi di era globalisasi saat ini, khususnya di bidang pelayanan kesehatan. Klinik Rini Eka Putri merupakan salah satu fasilitas pelayanan kesehatan yang menyelenggarakan upaya kesehatan masyarakat. Dalam kegiatan pelayanannya dalam mencatat rekam medis pasien belum maksimal karena masih dilakukan secara sederhana dan belum terkomputerisasi, yaitu dengan menggunakan media kertas. Program ini menggunakan bahasa pemograman PHP dan MySQL. Kata Kunci: Medical Record, Information System, Waterfall, PHP, MySQL
