Abstract

The purpose of this thesis is to design a method of application that can view thelocation of health centers and health care centers in each village or district in West Jakarta, so that people can more quickly find information about the nearest health center. The methodology used is waterfall method, method analysisdone with literature study, survey of relevant gencies, interview, and identification of data requirements for the system, method of design is done by designing use case, activity diagram, sequence siagram, menu layer, database, program specifications. The coding is done by using the help program of ASP.NET testing is done to assure that the output generatedprogram has been as expected. Results obtained the form of information center concluded that this applications canbe used by the Departement of health and Society to obtain information centers in West Jakarta, and also be used as input to improve the coordination between community an government. Keywords: Health Services Application, Based Map Tujuan penulisan skripsi ini adalah untuk merancang suatu metode aplikasi yang dapat melihat lokasi puskesmas dan pelayanan kesehatan puskesmas pada tiap kelurahan atau kecamatan yang ada di Jakarta Barat, sehingga masyarakat dapat lebih cepat mengetahui informasi mengenai puskesmas terdekat. Metodologi yang digunakan adalah metode waterfall, metode analisa dilakukan dengan studi kepustakaan, survey di instansi terkait, wawancara, dan identifikasi kebutuhan data bagi system, metode perancangan dilakukan dengan merancang USE CASE, ACTIVITY DIAGRAM, SEQUENCE DIAGRAM menu layer, basis data, spesifikasi program. Pengkodean dilakukan dengan menggunakan bantuan program ASP.NET Pengujian dilakukan untuk meyakinkan bahwa keluaran yang dihasilkan program telah sesuai dengan yang diharapkan. Hasil yang diperoleh berupa informasi puskesmas disimpulkan bahwa aplikasi ini dapat digunakan oleh Dinas Kesehatan dan Masyarakat untuk memperoleh informasi puskesmas di Jakarta Barat, dan dapat juga digunakan sebagai bahan masukan untuk meningkatkan koordinasi antara masyarakat dan pemerintah.
