Abstract

PT. Usaha Saudara Mandiri is a company that produces industrial heating elements. One of the products produced is a cartridge heater. The problem is that the number of defects in the production of cartridge heaters exceeds the standard set by the company by 3%. This study aims to find out the cause of disability and identify activities that have the highest level of risk to be the cause of disability to be done priority handling. The method used in the study is Fuzzy FMEA. The study used 4 expert respondents in the department of quality control and production. In the production of cartridge heaters there are 9 types of defects that occur, short heaters become the most dominant type of defect with a percentage of 89.8% caused by 4 factors namely humans, machines, working methods and work environment. Based on calculations using fuzzy FMEA obtained results that have the highest level of risk to be the cause of defects, namely the operator is less careful when the engine setting and engine performance is not good with a FRPN value of 788,333 which falls into the category of high-very high (H-VH) and becomes a top priority to make improvements. Salah satu produk yang diproduksi yaitu cartridge heater. Permasalahan yang terjadi adalah jumlah cacat pada produksi cartridge heater melebihi dari standar yang ditetapkan perusahaan sebesar 3%. Penelitian ini bertujuan untuk mengetahui penyebab terjadinya cacat serta mengidentifikasi aktivitas yang memiliki tingkat resiko tertinggi menjadi penyebab terjadinya cacat untuk dilakukan prioritas penanganan. Metode yang digunakan dalam penelitian yaitu Fuzzy FMEA. Penelitian ini menggunakan 4 responden ahli pada departemen quality control dan produksi. Pada produksi cartridge heater terdapat 9 jenis cacat yang terjadi, heater short menjadi jenis cacat yang paling dominan dengan presentase 89,8% yang disebabkan oleh 4 faktor yaitu manusia, mesin, metode kerja dan lingkungan kerja. Berdasarkan perhitungan menggunakan fuzzy FMEA didapatkan hasil yang memiliki tingkat resiko tertinggi menjadi penyebab terjadinya cacat yaitu operator kurang teliti saat setting mesin dan performa mesin kurang baik dengan nilai FRPN 788,333 yang termasuk dalam kategori high-very high (H-VH) dan menjadi prioritas utama untuk dilakukan perbaikan.
