Abstract
The use of the DMAIC method has proven to have a significant impact on quality control. PT. XYZ is a garment company. PT. XYZ manufactures various types of clothing such as clothes, shirts, jackets, alma mater, and others. One of the obstacles faced in the company's production process is the high defect that exceeds the target limit of 1% for button defect defects, and 5% for types of embroidered defects. During the period September 2018 - November 2018 the average button defects reached 3%, and embroidery defects reached 7.4%, this became a problem because the target limit for button defects was 1% and embroidery defects were 5%. Then it was also known that embroidery defects were the dominant defect in the period September 2018 - November 2018. After knowing the causes of defect embroidery defects using fishbone diagrams, then using the FMEA tool it is known that incorrect name input was ranked first which needs to be corrected. From the results of the 5W + 1H analysis it was found that suggestions for improvement were obtained from the results of discussions with the company and observations. Keywords: Defect, Failure Mode And Effect Analysis (FMEA), Fishbone. DMAIC, Pareto Diagram. Penggunaan metode DMAIC terbukti dapat memberikan dampak yang cukup signifikan dalam pengendalian kualitas. PT. XYZ adalah perusahaan yang bergerak di bidang garmen. PT. XYZ memproduksi berbagai jenis pakaian seperti baju, kemeja, jaket, almamater, dan lainnya. Salah satu hambatan yang dihadapi pada proses produksi perusahaan adalah tingginya defect yang melewati batas target yaitu 1% untuk jenis defect cacat kancing, dan 5% untuk jenis defect cacat bordir. Selama periode September 2018 – November 2018 rata-rata cacat kancing mencapai 3%, dan cacat bordir mencapai 7,4%, hal ini menjadi sebuah masalah karena batas target untuk cacat kancing adalah 1% dan cacat bordir adalah 5%. Lalu diketahui juga bahwa cacat bordir merupakan defect dominan pada periode September 2018 – November 2018. Setelah diketahui faktor penyebab defect cacat bordir dengan menggunakan diagram fishbone, selanjutnya dengan menggunakan tool FMEA diketahui bahwa salah input nama menempati peringkat pertama yang perlu diperbaiki. Dari hasil analisis 5W + 1H diketahui saran-saran perbaikan yang didapatkan dari hasil diskusi dengan pihak perusahaan dan pengamatan.
