Abstract
This study aims to analyze the causes of dominant defects and formulate improvement proposals for the production process of the Wedge Clamp 4x6–25 mm at an electrical component manufacturing company. Production data from 2025 shows a total output of 1,371,239 units with 36,861 defective units; in several periods, the defect rate exceeded the company's tolerance limit of 2.5%. The study employs a quantitative approach utilizing Root Cause Analysis (RCA) and Failure Mode and Effects Analysis (FMEA). Quality Control Tools were used to identify the dominant defect types, RCA to determine the root causes, and FMEA to prioritize risks based on Risk Priority Number (RPN) values. The Pareto Diagram indicates that cracking is the most dominant defect, accounting for 25,362 units or 68.8% of total defects. FMEA results highlight three causes with the highest RPN values: hydraulic pressure fluctuations (420), suboptimal material mixture control (324), and suboptimal furnace temperature regulation (252). Based on these findings, improvement proposals were formulated using a 5W+1H matrix, focusing on process parameter control through hydraulic pressure monitoring, material composition control, and furnace temperature automation. Implementing these improvements is expected to reduce process variation and lower the defect rate to below the company's 2.5% target. Keywords: Quality Control, RCA, FMEA, 5W+1H Penelitian ini bertujuan untuk menganalisis penyebab cacat dominan serta merumuskan usulan perbaikan pada proses produksi produk Wedge Clamp 4x6–25 MM di Industri Komponen Listrik. Berdasarkan data produksi selama tahun 2025, tercatat total produksi sebanyak 1.371.239 pcs dengan jumlah cacat mencapai 36.861 pcs, yang pada beberapa periode melebihi batas toleransi cacat perusahaan sebesar 2,5%. Penelitian menggunakan pendekatan kuantitatif dengan metode Root Cause Analysis (RCA), dan Failure Mode and Effects Analysis (FMEA). Quality Control Tools digunakan untuk mengidentifikasi jenis cacat dominan, RCA untuk menentukan akar penyebab terjadinya cacat, sedangkan FMEA digunakan untuk menentukan prioritas risiko berdasarkan nilai Risk Priority Number (RPN). Hasil Diagram Pareto menunjukkan bahwa cacat crack merupakan jenis kecacatan yang paling dominan dengan jumlah 25.362 pcs atau sebesar 68,8% dari total cacat. Hasil FMEA menunjukkan tiga penyebab dengan nilai RPN tertinggi, yaitu fluktuasi tekanan hidrolik (420), pengontrolan campuran material yang belum optimal (324), dan regulasi temperatur furnace yang belum optimal (252). Berdasarkan hasil tersebut, disusun usulan perbaikan menggunakan matriks 5W+1H yang berfokus pada pengendalian parameter proses melalui monitoring tekanan hidrolik, pengendalian komposisi material, dan otomatisasi temperatur furnace. Implementasi usulan perbaikan diharapkan mampu mengurangi variasi proses serta menurunkan tingkat cacat hingga berada di bawah target perusahaan sebesar 2,5%.
