Abstract

PT. Surya Toto Indonesia Tbk. is a manufacturing company engaged in the production of sanitary ware, fittings, and kitchen systems. One of the major challenges faced by the company is the occurrence of defective products in the Body Shower Mixer production process, which affects product quality, production efficiency, and manufacturing costs. This study aims to identify the most dominant defect types, analyze the factors contributing to product defects, and propose improvement strategies to reduce the defect rate and enhance product quality. The research employed the DMAIC (Define, Measure, Analyze, Improve, Control) methodology in combination with Failure Mode and Effect Analysis (FMEA). The data from the production and defect records of Body Shower Mixer products during January, February, March, April, May, June, August, September, October, and December 2025. During this period, a total of 16,331 units were produced, with 881 defective units recorded. The Define phase identified the main quality problems, the Measure phase evaluated the defect level using Pareto charts and P control charts, the Analyze phase determined the root causes through cause-andeffect diagrams and FMEA, while the Improve and Control phases focused on developing and sustaining corrective actions. The results indicate that Yogore (dirty contamination) was the most dominant defect, accounting for 421 units (47.79%), followed by Kizu (scratch defects) with 329 units (37.34%). Cause-andeffect analysis identified six major contributing factors: Man, Machine, Material, Method, Environment, and Measurement. The FMEA results revealed that dusty machines or work equipment had the highest Risk Priority Number (RPN) of 294, followed by inadequate machine maintenance with an RPN of 210. Recommended improvements include implementing regular machine cleaning schedules, using machine cleanliness checklists, conducting preventive maintenance, and performing routine workplace cleanliness audits and inspections. These improvements are expected to reduce product defects and improve the overall quality products. Keywords: DMAIC, FMEA, Quality Control PT. Surya Toto Indonesia Tbk. merupakan perusahaan manufaktur yang memproduksi sanitary ware, fitting, dan kitchen system. Salah satu permasalahan yang dihadapi perusahaan adalah masih ditemukannya produk cacat pada proses produksi Body Shower Mixer yang berdampak pada penurunan kualitas produk dan peningkatan biaya produksi. Penelitian ini bertujuan untuk mengidentifikasi jenis cacat yang paling dominan, menganalisis faktor-faktor penyebab terjadinya cacat, serta memberikan usulan perbaikan untuk menekan tingkat kecacatan produk. Metode yang digunakan adalah DMAIC (Define, Measure, Analyze, Improve, Control) dan Failure Mode and Effect Analysis (FMEA). Data penelitian diperoleh dari data produksi dan data cacat produk Body Shower Mixer selama Januari, Februari, Maret, April, Mei, Juni, Agustus, September, Oktober, dan Desember 2025, dengan total produksi sebanyak 16.331 unit dan jumlah produk cacat sebanyak 881 unit. Tahap Define dilakukan untuk mengidentifikasi permasalahan utama, Measure untuk mengukur tingkat kecacatan menggunakan diagram Pareto dan peta kendali P, Analyze untuk mengidentifikasi akar penyebab cacat melalui diagram sebab-akibat dan FMEA, Improve untuk menyusun usulan perbaikan, serta Control untuk mempertahankan hasil perbaikan. Hasil penelitian menunjukkan bahwa cacat Yogore (kotor) merupakan jenis cacat yang paling dominan, yaitu sebanyak 421 unit (47,79%), diikuti cacat Kizu (luka gores) sebanyak 329 unit (37,34%). Analisis diagram sebab-akibat mengidentifikasi enam faktor penyebab utama, yaitu Man, Machine, Material, Method, Environment, dan Measurement. Hasil FMEA menunjukkan bahwa mesin atau alat kerja yang berdebu memiliki nilai Risk Priority Number (RPN) tertinggi sebesar 294, diikuti kurangnya perawatan mesin dengan nilai RPN sebesar 210. Usulan perbaikan meliputi penerapan jadwal pembersihan mesin secara berkala, penggunaan checklist kebersihan mesin, pelaksanaan preventive maintenance, serta audit dan inspeksi kebersihan area kerja secara rutin. Implementasi usulan tersebut diharapkan mampu mengurangi jumlah produk cacat serta meningkatkan kualitas produk Body Shower Mixer. Kata Kunci: DMAIC, FMEA, Pengendalian Kualitas
