Abstract
A textile company is a manufacturing industry that produces Polyester Filament Yarn (PFY) type E900 as a raw material for satin fabrics and car seat upholstery. During the period of January–December 2025, quality problems were identified in the form of a product defect rate exceeding the company’s standard limit of 3.50%, resulting in operational losses. This study aims to analyze quality control efforts to reduce product defects. The methods used include Failure Mode and Effect Analysis (FMEA) to determine risk priorities based on the Risk Priority Number (RPN), Fault Tree Analysis (FTA) to identify the root causes of failures, and the 5W+1H approach to develop improvement recommendations. Data were collected through observations, interviews with operators, production leaders, and PPC supervisors, as well as documentation of production data and defect quantities. The results showed that the dominant defect in PFY E900 products was Ayaochi, which is a condition where one or more filaments come out from the yarn package, contributing 51% of the total defects. Based on the FMEA analysis, the three failure causes with the highest RPN values were non-standard travers installation (RPN 270), defective travers conditions (RPN 240), and improper winder machine settings (RPN 200). The FTA analysis revealed that the main causes of defects were inadequate operator skills, inconsistent SOP implementation, component wear, and suboptimal preventive maintenance activities. The proposed improvements include operator training, implementation of SOPs and work checklists, routine inspection of travers conditions, and enhancement of winder machine maintenance. The implementation of these improvements is expected to reduce defect rates, improve product quality consistency, and support the operational efficiency of the textile company. Keywords: Quality Control, FMEA, FTA, 5W+1H Perusahaan tekstil merupakan industri manufaktur yang memproduksi Polyester Filament Yarn (PFY) tipe E900 sebagai bahan baku kain satin dan jok mobil. Pada periode Januari–Desember 2025, ditemukan permasalahan kualitas berupa tingkat cacat produk yang melebihi standar perusahaan sebesar 3,50%, sehingga menyebabkan kerugian operasional. Penelitian ini bertujuan untuk menganalisis pengendalian kualitas guna menurunkan tingkat kecacatan produk. Metode yang digunakan yaitu Failure Mode and Effect Analysis (FMEA) untuk menentukan prioritas risiko berdasarkan nilai Risk Priority Number (RPN), Fault Tree Analysis (FTA) untuk mengidentifikasi akar penyebab kegagalan, serta pendekatan 5W+1H untuk menyusun usulan perbaikan. Data penelitian diperoleh melalui observasi, wawancara dengan operator, leader produksi, dan supervisor PPC, serta dokumentasi data produksi dan jumlah kecacatan. Hasil penelitian menunjukkan bahwa cacat dominan pada produk PFY E900 adalah Ayaochi, yaitu kondisi satu atau beberapa helai filament keluar dari gulungan, dengan kontribusi sebesar 51% dari total cacat. Berdasarkan analisis FMEA, tiga penyebab dengan nilai RPN tertinggi yaitu pemasangan travers tidak sesuai standar (RPN 270), kondisi travers cacat (RPN 240), dan pengaturan mesin winder yang tidak optimal (RPN 200). Hasil analisis FTA menunjukkan bahwa penyebab utama cacat berasal dari keterampilan operator yang belum optimal, penerapan SOP yang belum konsisten, keausan komponen mesin, serta kegiatan preventive maintenance yang belum maksimal. Usulan perbaikan yang diberikan meliputi pelatihan operator, penerapan SOP dan checklist kerja, pemeriksaan rutin kondisi travers, serta peningkatan perawatan mesin winder. Implementasi perbaikan tersebut diharapkan mampu menurunkan tingkat cacat, meningkatkan konsistensi kualitas produk, dan mendukung efisiensi operasional perusahaan tekstil.
