Abstract
Workplace accidents in the manufacturing industry remain a serious issue, including in cold room production processes involving sharp metal materials, welding processes, and chemicals. Based on work accident data for July–December 2025, there were 16 accident cases dominated by lacerations in bending and cutting processes, as well as UV exposure in welding. When an accident occurs, production experiences downtime because other workers must assist the victim, while the company does not yet have a dedicated OHS team. This study aims to identify potential hazards, assess risk levels, analyze the root causes of dominant risks, and propose risk control recommendations in the refrigeration equipment manufacturing industry. The methods used are HIRARC and FTA. Data were obtained through observation, interviews, and brainstorming involving 7 Head Operators and 1 Factory Supervisor across 13 production processes. The results identified 80 potential hazards grouped into 12 types of risks. Risk assessment resulted in 14 Low, 32 Moderate, 26 High, and 8 Extreme risks. The highest priority risks were eye damage due to UV exposure (score 12) and electric shock (score 10). FTA analysis revealed that the root causes of dominant risks came from management factors, worker behavior factors, and technical factors. Management factors include lack of SOPs, supervision, OHS training, and procurement priority. Worker behavior factors include low risk perception, non-compliance with PPE, and reactive culture. Technical factors include difficult-to-reach welding positions, high work intensity, and worn components. Risk control recommendations were developed based on the hierarchy of controls including engineering controls, substitution, administrative controls, and standard PPE. The implementation of these recommendations is expected to reduce the level of workplace accident risks and improve occupational safety in the production process. Keywords: occupational safety, HIRARC, Fault Tree Analysis, risk, hazard, work accidents Kecelakaan kerja pada industri manufaktur masih menjadi permasalahan serius, termasuk pada proses produksi cold room yang melibatkan material logam tajam, proses pengelasan, dan bahan kimia. Berdasarkan data kecelakaan kerja periode Juli–Desember 2025, terjadi 16 kasus kecelakaan yang didominasi oleh luka gores pada proses bending dan cutting, serta paparan sinar UV pada proses pengelasan. Ketika kecelakaan terjadi, proses produksi mengalami downtime karena pekerja lain harus membantu korban, sementara perusahaan belum memiliki tim K3 khusus. Penelitian ini bertujuan untuk mengidentifikasi potensi bahaya, menilai tingkat risiko, menganalisis akar penyebab risiko dominan, serta menyusun usulan pengendalian risiko pada industri manufaktur peralatan pendingin. Metode yang digunakan adalah HIRARC dan FTA. Data diperoleh melalui observasi, wawancara, dan brainstorming dengan 7 Kepala Operator dan 1 Supervisor Factory pada 13 proses produksi. Hasil penelitian menunjukkan 80 potensi bahaya yang dikelompokkan ke dalam 12 jenis risiko. Penilaian risiko menghasilkan 14 Low, 32 Moderate, 26 High, dan 8 Extreme. Risiko prioritas tertinggi adalah kerusakan mata akibat sinar UV (skor 12) dan sengatan listrik (skor 10). Hasil FTA menunjukkan akar penyebab risiko dominan berasal dari faktor manajemen, perilaku pekerja, dan teknis. Faktor manajemen meliputi kurangnya SOP, pengawasan, pelatihan K3, dan prioritas pengadaan. Faktor perilaku pekerja meliputi rendahnya persepsi risiko, ketidakpatuhan APD, dan budaya reaktif. Faktor teknis meliputi posisi pengelasan sulit dijangkau, intensitas pekerjaan tinggi, dan komponen aus. Usulan pengendalian disusun berdasarkan hierarki kontrol meliputi rekayasa teknik, substitusi, administratif, dan APD. Implementasi usulan tersebut diharapkan dapat menurunkan tingkat risiko kecelakaan kerja dan meningkatkan keselamatan kerja pada proses produksi.
