Abstract
This study aims to analyze risks in the management of used gallon water bottles within a reverse logistics system using the Failure Mode and Effects Analysis (FMEA) and House of Risk (HOR) methods. The study variables include potential failure modes and associated risk agent affecting the gallon return process. A total of 48 respondents, who are regular customers of a refillable water depot in Sunter, North Jakarta, were selected through purposive sampling, with a minimum of six months of usage experience. Data were collected via questionnaires, interviews, field observations, and literature review. The analysis involved calculating the Risk Priority Number (RPN) using FMEA and identifying dominant causes and mitigation strategies based on the Effectiveness to Difficulty Ratio (ETD) using HOR. The results showed that the highest risk failure modes were lack of communication (RPN = 67.0), depot responsibility (RPN = 65.9), and delay in return processes (RPN = 64.7). The most significant risk agent was the lack of staff training (RA3), with an ARP of 143.95. The top recommended mitigation strategies include formalization of standard operating procedure (SOP) for returns with an ETD value of 576.29, regular operational staff training (ETD = 431.85), and the implementation of automated customer reminder systems (ETD = 402.06). The study concludes that the integration of FMEA and HOR is effective for identifying, prioritizing, and formulating efficient and measurable risk mitigation strategies in gallon bottle reverse logistics. Keywords: Reverse Logistics, FMEA, House of Risk, Mitigation Strategy Penelitian ini bertujuan untuk menganalisis risiko dalam pengelolaan limbah galon air minum pada sistem reverse logistics menggunakan metode Failure Mode and Effects Analysis (FMEA) dan House of Risk (HOR). Variabel penelitian mencakup potensi kegagalan (failure mode) dan penyebab risiko (risk agent) dalam proses pengembalian galon bekas. Sebanyak 48 responden konsumen depot air minum isi ulang di wilayah Sunter, Jakarta Utara dipilih menggunakan teknik purposive sampling dengan kriteria minimal enam bulan sebagai pelanggan aktif. Pengumpulan data dilakukan melalui kuesioner, wawancara, observasi, dan studi pustaka. Analisis dilakukan melalui perhitungan Risk Priority Number (RPN) menggunakan FMEA, serta identifikasi penyebab dominan dan strategi mitigasi berdasarkan Effectiveness to Difficulty Ratio (ETD) menggunakan HOR. Hasil penelitian menunjukkan bahwa failure mode dengan nilai RPN tertinggi adalah kurangnya komunikasi (RPN = 67,0), tanggung jawab depot (RPN = 65,9), dan keterlambatan proses pengembalian (RPN = 64,7). Risk agent paling dominan adalah kurangnya pelatihan staf (RA3) dengan nilai ARP sebesar 143,95. Strategi mitigasi yang paling direkomendasikan meliputi penyusunan SOP formal pengembalian galon dengan nilai ETD sebesar 576,29, pelatihan rutin staf operasional (ETD = 431,85), dan sistem pengingat otomatis kepada pelanggan (ETD = 402,06). Penelitian ini menyimpulkan bahwa metode FMEA dan HOR efektif dalam mengidentifikasi, memprioritaskan, dan menyusun strategi mitigasi risiko yang efisien dan terukur.
