Abstract

Construction project development is known as a high-risk activity, including beam and plate formwork that is within the scope of the project activity. Occupational Health and Safety (OHS) is very important for construction companies, therefore it is important to know the sources of OHS risks that cause work accidents for prevention, for this it is necessary to have a risk management system, the HIRADC (Hazard Identification Risk Determining Control) method is carried out withidentifying hazards then assessing the risk of hazards at the work location, then making a control plan for these risks, the Fuzzy DEMATEL (Decision Making Trial and Evaluation Laboratory) serves to determine the main OSH risk sources that are prioritized and evaluate the probability/frequency and the greatest possible impact with a cause-and-effect diagram. From research on the Depok DLP Warehouse Project (Depok Logistic Property) on beam and slab formwork, 3 main OHS risk sources were obtained with 5 OHS hazard potentials, based on the highest coordinate point on the cause and effect diagram, the first is PCH installation with a potential OHS hazard of falling from a height, slipped from a height and fell from a height, then installation of bracing pipes on the PCH with a potential OHS hazard from falling from a height and demolition of beam and slab formwork with a potential OHS hazard of falling from a height. Risk control of the main OHS risk sources recommended is engineering, administration and the use of Personal Protective Equipment (PPE). Keywords: Risk, OHS, HIRADC, Fuzzy DEMATEL, Formwork Pembangunan proyek konstruksi di kenal sebagai kegiatan yang berrisiko tinggi,tidak terkecuali pekerjaan bekisting balok dan pelat yang ada pada lingkup kegiatan proyek.Keselamatan dan Kesehatan Kerja (K3) menjadi hal yang sangat penting bagi perusahaan konstruksi,oleh karena itu pentingnya mengetahui sumber risikio K3 penyebab kecelakaan kerja untuk dilakukan pencegahan ,untuk itu perlu adanya sistem manajemen risiko,metode HIRADC (Hazard Identification Risk Determining Control)dilakukan dengan mengidentifikasi bahaya selanjutnya menilai risiko dari bahaya pada lokasi pekerjaan, lalu membuat rencana pengendalian dari risiko tersebut,metode Fuzzy DEMATEL (Decision Making Trial and Evaluation Laboratory) berfungsi untuk menentukan sumber risiko K3 utama yang menjadi prioritas dan mengevaluasi kemungkinan probabilitas/frekuensi dan dampak terbesar dengan diagram sebab akibat. Dari penelitian pada Proyek Warehouse DLP Depok(Depok Logistic Property ) pada pekerjaan bekisting balok dan pelat di daptakan 3 sumber risiko K3 utama dengan dengan 5 potensi bahaya K3, berdarkan titik koordinat tertinggi pada diagram sebab akibat, yang pertama pemasanagn PCH dengan potensi bahaya K3 terjatuh dari ketinggian, terpeleset dari ketinggian dan terperosok dari ketinggian, berikutnya pemasangan pipa bracing pada PCH dengan potensi bahaya K3 terjatuh dari ketinggian dan pembongkaran bekisting balok dan pelat dengan potensi bahaya K3 terjatuh dari ketinggian.Pengendalian risiko sumber risiko K3 utama yang di rekomendasikan adalah rekayasa teknik (Engineering), administrasi (Administrative), dan penggunaan Alat Pelindung Diri (APD).
