Abstract
This study aims to analyze work accidents in construction projects. With the aim of reducing the impact or risk of work accidents during project implementation. This research uses risk management that combines two tools, namely the FMEA method and the fishbone diagram. After calculating the FMEA, the RPN value that exceeds the critical value, namely being hit by a sharp object during installation of iron and formwork, falling from a height when casting concrete, being pinched during the mobilization of concrete material, damage due to work tools during iron and formwork, falling from a the same when working on steel construction, fire due to welding during the installation of air pipes, tripping / slipping at the work site during iron and formwork installation. Based on the analysis, work accidents are often during structural work. Based on the fishbone analysis, the causes of accidents are not being careful when working, workers not wearing PPE, not optimal implementation of HSE work operational standards, lack of HSE officers, no safety system on machines, lack of concern for cleanliness. Improvements made are the implementation of workforce training accompanied by the National Agency for Professional Certification, updating work operational standards by checking the conditions of the work environment before work is carried out, making a one gate system as access to and from workers to facilitate screening of worker PPE, changes to project operational management structures, renewal of tools with a safer work system and create a project environmental cleaning cleanliness program that involves all workers. Keywords: FMEA, fishbone diagram, work accidents, HSE Penelitian ini bertujuan menganalisis penyebab kecelakaan kerja di proyek konstruksi. Dengan tujuan untuk mengurangi dampak atau resiko kecelakaan kerja pada saat pelaksanaan proyek. Penelitian ini menggunakan manajemen risiko yang menggabungkan dua alat yaitu metode FMEA dan fishbone diagram. Setelah dilakukan penghitungan FMEA, nilai RPN yang melebihi nilai kritis yaitu terkena benda tajam pada saat pemasangan besi dan bekisting, terjatuh dari ketinggian saat pengecoran beton, terjepit pada saat mobilisasi material material beton, kecelakaan akibat alat kerja selama besi dan bekisting, terjatuh dari ketinggian yang sama saat mengerjakan konstruksi besi, kebakaran akibat pengelasan pada saat pemasangan pipa air, tersandung/tergelincir di lokasi kerja pada saat pemasangan besi dan bekisting. Berdasarkan analisis, kecelakaan kerja sering terjadi pada saat pekerjaan struktural. Berdasarkan analisa fishbone, penyebab terjadinya kecelakaan yaitu kurang hati-hati saat bekerja, pekerja tidak memakai APD, penerapan SOP K3 tidak maksimal, kurangnya petugas K3, tidak ada safety sistem pada mesin, kurangnya kepedulian akan kebersihan. Perbaikan yang dilakukan adalah pelaksanaan pelatihan tenaga kerja didampingi oleh Badan Nasional Sertifikasi Profesi, pemabaharuan SOP dengan melakukan pengecekan kondisi lingkungan kerja sebelum pekerjaan dilakukan, membuat one gate system sebagai akses keluar masuk pekerja untuk memudahkan screening APD pekerja, perubahan struktur manajemen operasional proyek, pembaharuan alat dengan sistem kerja yang lebih aman dan membuat program kebersihan pembersihan lingkungan proyek yang melibatkan seluruh pekerja.
