Abstract
PT BF is a company engaged in the automotive industry. The product produced one of them is brake fluid. The production process of brake fluid includes the process of raw material mixer, filling process, capping process, printing process, sealing process, and packing process. The results of product defect calculations that occurred in the company still exist that are beyond the limit set by the company that is equal to 3%. Therefore, the company must have to immediately evaluate and improve on the production process in progress. The tools used in this research are Failure Mode and Effects Analysis (FMEA) method to identify and analyze the failure that occurs during production process. The purpose of this study is to analyze the types and causes of defects, and provide suggestions for improvement to reduce product defects. The final result of the value of Risk Priority Number (RPN) is obtained by determining and multiplying the severity, occurrence, and failure detection. Based on the results of the research, the largest RPN value that became the priority was the color of the oil that changed due to the mixed and slightly cleaned amount 192. The cause of product failure is due to engine, operator error, and supplier quality. Keywords : Product failure, product defect, brake fluid, failure mode and effects analysis, risk priority number PT BF merupakan perusahaan yang bergerak dibidang industri otomotif. Produk yang dihasilkan salah satunya adalah minyak rem. Proses produksi minyak rem meliputi proses mixer bahan baku, proses filling, proses capping, proses printing, proses sealing, dan proses packing. Hasil perhitungan cacat produk yang terjadi di perusahaan masih ada yang berada diluar batas yang telah ditetapkan oleh perusahaan yaitu sebesar 3%. Oleh karena itu perusahaan harus harus segera melakukan evaluasi dan perbaikan terhadap proses produksi yang berlangsung. Tools yang digunakan dalam penelitian ini adalah metode Failure Mode and Effects Analysis (FMEA) untuk mengidentifikasi dan menganalisa kegagalan yang terjadi selama proses produksi. Tujuan penelitian ini adalah menganalisa jenis dan penyebab cacat, serta memberikan usulan perbaikan untuk mengurangi kecacatan produk. Hasil akhir berupa nilai Risk Priority Number (RPN) diperoleh dengan menentukan dan mengalikan tingkat keparahan, kejadian, dan deteksi kegagalan. Berdasarkan hasil penelitian, nilai RPN terbesar yang menjadi prioritas perbaikan adalah warna minyak rem yang berubah karena selang yang bercampur dan jarang dibersihkan sebesar 192. Penyebab kegagalan produk disebabkan karena mesin, kesalahan operator, dan kualitas supplier.
