Abstract

Peraturan Menteri Kesehatan No. 1 tahun 2012 tentang Sistem Rujukan Pelayanan Kesehatan merupakan langkah awal untuk memberikan pelayanan kesehatan yang effektif dan effisien. Penelitian ini bertujuan untuk melihat perbedaan jumlah kunjungan, rujukan dan biaya pelayanan kesehatan sebelum dan sesudah diterapkannya kebijakan sistem rujukan berjenjang. Populasi penelitian ini adalah Puskesmas di wilayah Jakarta Barat dengan sampel 59 puskesmas yang melayani peserta PT. Askes (Persero) tahun 2011 s/d 2013. Rancangan penelitian ini menggunakan studi kausal komparatif, yaitu mengkaji sebelum dan sesudah kebijakan diterapkan dan uji hipotesis menggunakan paired sample t test. Data penelitian menggunakan data sekunder periode sebelum dan sesudah kebijakan dikeluarkan (2011, 2012 dan 2013). Hasil Penelitian menunjukkan bahwa tidak ada perbedaan signifikan pada jumlah kunjungan sebelum dan sesudah diterapkannya kebijakan sistem rujukan berjenjang sehingga dapat dikatakan bahwa menurunnya jumlah kunjungan di fasilitas kesehatan tingkat pertama (puskesmas) tidak menyebabkan menurunnya biaya pelayanan kesehatan. Sedangkan terdapat perbedaan signifikan pada jumlah rujukan sebelum dan sesudah diterapkannya sistem rujukan berjenjang dimana peserta tidak dapat langsung ke rumah sakit yang tipenya lebih tinggi. Penurunan biaya pelayanan obat di rumah sakit terjadi pada tahun 2012 tetapi tidak berpengaruh signifikan terhadap biaya pelayanan kesehatan secara keseluruhan dikarenakan masih meningkatnya jumlah kunjungan peserta di rumah sakit khusus. This study aims to look at the differences in the number of total visits, total referral and health care costs before and after implementation of a tiered referral system policy. The population was health center in West Jakarta with samples 59 health centers that serve participants PT. Askes (Persero) in 2011 s / d 2013. The study design using causal comparative studies, including reviewing before and after the policy is applied and hypothesis using paired- sample test. The research uses secondary data before and after the policy was issued (2011, 2012 and 2013). Research shows that were no significant differences in the number of visits before and after the implementation of policies tiered referral system so that it can be said that the decline in the number of visits in the first-level health facilities (health centers) do not lead to decreased health care costs. While there are significant differences in the number of referrals before and after implementation of a tiered referral system where participants can not directly to the hospital whose type is higher.
