Abstract

PT Biggy Cemerlang is a manufacturing company that produces plastic packaging using the injection molding process. During the period December 2024 to February 2025, the defect rate of CT 156 products was found to exceed the company's tolerance threshold of <10%, with the most common defect type being flashes which accounted for 42.26% of the total defects. This study aims to identify the dominant defect type, analyze the root cause, and provide improvement proposals to reduce the defect rate. The method used is Six Sigma approach with DMAIC (Define, Measure, Analyze, Improve, Control) stages. The analysis tools used include check sheets, Pareto diagrams, fishbone diagrams, FMEA (Failure Mode and Effect Analysis), P control maps, and the 5W+1H method. The analysis results show that the DPMO value is 25,000 with a sigma level of 3.46, indicating that the process has not reached the Six Sigma standard. FMEA revealed that the main cause of flashes defects comes from human factors, namely lack of operator training and process setup errors. Proposed improvements include a training program for injection molding machine technicians, increased production supervision, and regular audits of SOP implementation. With the implementation of these corrective actions, it is expected that the number of defects can be reduced and the quality of CT 156 products improved sustainably. PT Biggy Cemerlang merupakan perusahaan manufaktur yang memproduksi kemasan plastik menggunakan proses injection molding. Selama periode Desember 2024 hingga Februari 2025, ditemukan tingkat cacat produk CT 156 yang melebihi ambang batas toleransi perusahaan sebesar <10%, dengan jenis cacat terbanyak adalah flashes yang menyumbang 42,26% dari total cacat. Penelitian ini bertujuan untuk mengidentifikasi jenis cacat dominan, menganalisis akar penyebabnya, dan memberikan usulan perbaikan guna menurunkan defect rate. Metode yang digunakan adalah pendekatan Six Sigma dengan tahapan DMAIC (Define, Measure, Analyze, Improve, Control). Alat bantu analisis yang digunakan meliputi check sheet, diagram Pareto, fishbone diagram, FMEA (Failure Mode and Effect Analysis), peta kendali P, serta metode 5W+1H. Hasil analisis menunjukkan bahwa nilai DPMO sebesar 25.000 dengan level sigma 3,46, menandakan bahwa proses belum mencapai standar Six Sigma. FMEA mengungkap bahwa penyebab utama cacat flashes berasal dari faktor manusia, yaitu kurangnya pelatihan operator dan kesalahan pengaturan proses. Usulan perbaikan meliputi program pelatihan teknisi mesin injection molding, peningkatan pengawasan produksi, serta audit rutin terhadap pelaksanaan SOP. Dengan implementasi tindakan perbaikan ini, diharapkan jumlah cacat dapat ditekan dan kualitas produk CT 156 meningkat secara berkelanjutan.
