Abstract
In the basement floor structure work of Pusat Pelayanan Ibu dan Anak RS PIK Project, PIK 2, Jakarta Utara serious defects were found in the reinforced concrete, formwork, and rebar works, which could affect the project’s quality. The researcher conducted observations and analysis, applying the Six Sigma DMAIC method to measure and identify the causes of defects and how to control them. The research findings showed that the void defect in the basement column structure had the lowest sigma value of 2.3 with a DPMO of 200,000, still within the standard sigma level for construction projects in Indonesia, while the slab crack defect had the highest sigma value of 3.9, indicating good concrete quality. These defects were caused by several factors, such as ineffective work methods, weak inter-division communication, and aggressive environmental conditions. By applying Six Sigma DMAIC, the researcher successfully identified the causes of defects, formulated corrective solutions, and ensured the implementation of improvements. The main recommendation in this study is the addition of missing process flows to avoid further defects and improve the overall project quality. Key Words: Defect, Six Sigma, DMAIC, DPMO Pada pekerjaan struktur lantai basement Proyek Pusat Pelayanan Ibu dan Anak RS PIK, PIK 2, Jakarta Utara, ditemukan adanya cacat yang serius pada pekerjaan beton bertulang, bekisting, dan pembesian, yang dapat mempengaruhi kualitas pekerjaan proyek. Peneliti melakukan observasi dan analisis, serta menerapkan metode Six Sigma DMAIC untuk mengukur dan mengidentifikasi faktor penyebab cacat serta cara pengendaliannya. Hasil penelitian menunjukkan cacat keropos pada kolom struktur basement memiliki nilai sigma terendah 2,3 dengan DPMO 200.000 namun nilai tersebut masih berada pada batas standar level sigma proyek kontruksi di Indonesia, sementara cacat retak slab memiliki nilai sigma tertinggi 3,9, menunjukkan kualitas beton yang baik. Cacat pekerjaan tersebut disebabkan oleh beberapa faktor seperti metode kerja kurang efektif, komunikasi antar divisi lemah, dan kondisi lingkungan yang agresif. Dengan menerapkan Six Sigma DMAIC, peneliti berhasil mengidentifikasi penyebab cacat, merumuskan solusi perbaikan, dan memastikan perbaikan diterapkan. Rekomendasi utama pada penelitian ini adalah penambahan alur proses yang kurang untuk menghindari cacat lebih lanjut dan meningkatkan kualitas proyek secara keseluruhan.
