Abstract
PT. International Chemical Industry is well known worldwide, having exported its products in the form of dry, environmentally friendly batteries. PT. International Chemical Industry is very concerned about the quality of products it produces, not only that PT. International Chemical Industry is also very concerned about the work environment of the company. This is realized by the management of PT. International Chemical Industry with the application of ISO 9000 Quality Management System and ISO 14001 Environmental Management System. In July 1995 PT. International Chemical Industry successfully obtained ISO 9002: 1994 certificate from QAS (now SAI Global) certification body from Australia, then renewed in March 2003 to ISO 9001: 2000 from the same certification body. And in August 2009 renewed again to ISO 9001: 2008. The target defect the company wants is zero defect. This research uses Failure Mode and Effect Analysis (FMEA) method and Fault Tree Analysis (FTA) method to identify failure and provide preventive solution. The purpose of this study is to identify the types of defects that occur in battery products PT. International Chemical Industry based on literature study and deep interview with employees of PT. International Chemical Industry, is at risk of failure of the largest production process in the Risk Priority Number (RPN) value of the FMEA method which is then reanalyzed using the FTA method and determines the required repair proposal based on the defect analysis that occurs in the battery product. Based on data processing using FMEA and FTA method can identify failure mode that occurs in battery manufacturing process in PT. International Chemical Industry. International Chemical Industry sudah dikenal di seluruh dunia, karena telah mengekspor hasil produksinya berupa baterai kering yang ramah lingkungan. PT. International Chemical Industry sangat memperhatikan pada mutu produk yang dihasilkannya, tak hanya itu PT. International Chemical Industry juga sangat memperhatikan lingkungan kerja perusahaan. Ini diwujudkan oleh manajemen PT. International Chemical Industry dengan penerapan Sistem Manajemen Mutu ISO 9000 dan Sistem Manajemen Lingkungan ISO 14001. Pada bulan Juli 1995 PT. International Chemical Industry berhasil mendapatkan sertifikat ISO 9002:1994 dari QAS (sekarang SAI Global) lembaga sertifikasi dari Australia, kemudian diperbaharui pada bulan Maret 2003 menjadi ISO 9001:2000 dari lembaga sertifikasi yang sama. Dan pada bulan Agustus 2009 diperbaharui lagi menjadi ISO 9001 :2008. Target cacat produksi yang diinginkan perusahaan adalah zero defect. Dalam penelitian ini menggunakan metode Failure Mode and Effect Analysis (FMEA) dan metode Fault Tree Analysis (FTA) untuk mengidentifikasi kegagalan yang terjadi dan memberikan solusi pencegahan. Tujuan penelitian ini adalah Mengidentifikasi jenis-jenis cacat yang terjadi pada produk baterai PT. International Chemical Industry berdasarkan pada hasil penelusuran studi literatur dan deep interview dengan para karyawan dari PT. International Chemical Industry, mendapatkan resiko kegagalan proses produksi terbesar dalam nilai Risk Priority Number (RPN) dari metode FMEA yang kemudian dianalisis kembali menggunakan metode FTA dan menentukan usulan perbaikan yang diperlukan berdasarkan analisis defect yang terjadi pada produk baterai. Berdasarkan pengolahan data dengan menggunakan metode FMEA dan FTA dapat mengidentifikasi moda kegagalan yang terjadi pada proses pembuatan baterai di PT. International Chemical Industry.
