Abstract
PT. Indo Tirta Abadi is a manufacturing company engaged in plastic packaging, which produces PET Bottle, PET Preforms and HDPE CAPs. The problem that occurs with the bottle hot fill product is that the number of defects is quite high. Based on data taken from November 2022 to August 2023, the number of defects is as follows: Black spots 2488 pcs, Sylver body 2331 pcs, Body scratches 2162 pcs, Broken neck 6481 pcs and Dekok shoulders 2486 pcs. Based on the Pareto diagram, the highest defect is broken necks with a total of 6481 defects. So the main repair is focused on the broken neck defect using the Fault Tree Analysis (FTA) and Failure Mode and Effect Analysis (FMEA) methods. Indo Tirta Abadi merupakan salah satu perusahaan manufaktur yang bergerak dibidang plastic packaging, yang memproduksi Bottle PET, Preform PET dan CAP HDPE. Permasalahan yang terjadi pada produk bottle hot fill adalah jumlah defect yang cukup tinggi. Berdasarkan data yang diambil dari November 2022 s/d Agustus 2023 jumlah defect dengan rincian sebagai berikut: Bintik hitam 2488 pcs, Sylver body 2331 pcs, Gores body 2162 pcs, Neck pecah 6481 pcs dan Bahu dekok 2486 pcs. Berdasarkan diagram pareto defect tertinggi adalah neck pecah dengan total defect 6481 pcs. Sehingga perbaikan utama difokuskan pada defect neck pecah dengan menggunakan metode Fault Tree Analysisi (FTA) dan Failure Mode and Effect Analysis (FMEA).
