Abstract

Kegagalan merupakan permasalahan besar dalam proses manufaktur terutama produksi dalam jumlah besar dapat menurunkan produktivitas perusahaan disebabkan sering terjadinya kegagalan yang terdapat pada hasil produksinya. Alternative Material merupakan suatu sistem pengelolaan yang terintegrasi, berkesinambungan dan mencegah dalam menanggulangi pencemaran yang diakibatkan oleh Limbah B3 dari Pembangkit Listrik Tenaga Uap (PLTU), bertujuan untuk melakukan pemulihan kualitas lingkungan yang sudah tercemar sehingga sesuai dengan fungsinya kembali, data yang diperoleh di PT Multi Hanna Kreasindo Dept. QC R&D terdapat produk defect/kegagalan sebesar 5%. Metode FMEA (Failure Mode and Effect Analysis) membuat daftar potensi kegagalan produk melalui penilaian tiga aspek yaitu Tingkat kemungkinan frekwensi terjadi kegagalan (O=Occurence), Tingkat resiko akibat kegagalan (S=Severity) dan Tingkat kemungkinan bisa dideteksi (D=Detection). Dari hasil perhitungan dan analisis FMEA maka dihasilkan urutan prioritas potensi kegagalan dan kemungkinan penyebabnya, dampak dan solusinya. Nilai Risk Priority Number (RPN) ditugaskan untuk setiap masalah yang digunakan untuk membandingkan penyebab yang diidentifikasi dan 5W+1H untuk usulan perbaikan. Dari hasil penelitian menunjukkan bahwa faktor utama yang menyebabkan kegagalan dalam proses produksi yaitu : Defect sulfur 23%, Defect cl 10%, Defect moisture 32%, Defect TOC (Total Organik Carbon) 29% dan Defect Total Oksida. Berdasarkan Pareto Chart terbesar yaitu Defect Moisture 32%. Hasil analisa 5W+1H untuk usulan perbaikan produk lengket dan menggumpal dan komposisi produk tidak stabil dengan bobot nilai 128, dilakukan penambahan alat drying untuk mengurangi waktu proses mixing oleh Dep. R&D. Kata Kunci : Limbah B3, Alternative Material, FMEA, RPN Failure is a big problem in the manufacturing process, especially production in large quantities can reduce company productivity due to frequent failures contained in its production results. Alternative Material is an integrated, sustainable and preventative management system in overcoming pollution caused by Hazardouse Waste from Steam Power Plants (PLTU), aims to restore the quality of the polluted environment so that in accordance with its function again, the data obtained at PT Multi Hanna Kreasindo Dept. QC R&D there are defect products / failures of 5%. The FMEA (Failure Mode and Effect Analysis) method lists potential product failures through an assessment of three aspects, namely the level of probability of failure (O = Occurence), the level of risk due to failure (S = Severity) and the level of possibility that can be detected (D = Detection). From the results of the calculation and analysis of FMEA, a priority order is produced for potential failures and possible causes, impacts and solutions. A Risk Priority Number (RPN) value is assigned to each issue that is used to compare the identified causes and 5W+1H for proposed improvements. The results showed that the main factors that cause failure in the production process are: Defect sulfur 23%, Defect cl 10%, Defect moisture 32%, Defect TOC (Total Organic Carbon) 29% and Defect Total Oxide. Based on the Pareto Chart, the largest is Defect Moisture 32%. The results of the 5W+1H analysis for the proposed improvement of sticky and lumpy products and unstable product composition with a value weight of 128, added drying equipment to reduce the mixing process time by the Dep.
