Abstract

ABSTRAK PT Nandya Karya Perkasa adalah perusahaan yang bergerak dibidang otomotif yaitu produk part stay horn. Permasalahan yang terjadi yaitu jumlah defect pada produk part stay horn mengalami jumlah defect yang melebihi standar sebesar 2.06% serta standar yang ditetapkan oleh perusahaan sebesar 1%. Penelitian ini bertujuan untuk mengetahui penyebab defect tertinggi, mengetahui nilai RPN, dan memberikan usulan perbaikan untuk mengurangi jumlah defect sehingga memenuhi standar yang sudah ditetapkan perusahaan. Metode yang digunakan untuk penelitian ini yaitu FTA dan FMEA. Produk part stay horn terdapat 4 jenis defect seperti defect spatter, nat seret, welding tembus, dan welding keropos. Defect yang sering terjadi berdasarkan diagram pareto yaitu defect spatter sebesar 33%, nat seret 28%, welding tembus 20%, dan welding keropos 19%, serta faktor penyebab yang diketahui dengan menggunakan FTA terdapat 5 faktor penyebab seperti manusia, mesin, metode, lingkungan, dan material. Hasil identifikasi FMEA dan perhitungan RPN tertinggi, dapat diketahui bahwa kurang teliti dalam proses pengesalan sebasar 448, kinerja karyawan yang tidak sesuai SOP sebesar 294, dan elektroda lembab sebesar 252. Untuk usulan perbaikan yaitu memberikan pelatihan pada setiap operator dengan teori dan praktik pada proses welding dengan ketentuan yang sudah ditetapkan oleh perusahaan dan melakukan briefing sebelum melakukan pekerjaan dan setelah pekerjaan. The problem that occurs is that the number of defects in the stay horn part product has a number of defects that exceeds the standard of 2.06% and the standard set by the company is 1%. This study aims to determine the causes of the highest defects, determine the RPN value, and provide recommendations for improvements to reduce the number of defects so that they meet the standards set by the company. The methods used for this research are FTA and FMEA. The stay horn part product has 4 types of defects such as spatter defects, dragging grout, translucent welding, and porous welding. The defects that often occur based on the Pareto diagram are spatter defects of 33%, grout dragging 28%, welding penetration 20%, and porous welding 19%, and the causative factors known using FTA are 5 causal factors such as humans, machines, methods, environment , and materials. The results of the identification of FMEA and the highest RPN calculation, it can be seen that the less thorough in the process of regret is 448, the performance of employees who are not in accordance with the SOP is 294, and the electrode is moist by 252. For the proposed improvement, namely providing training to each operator with theory and practice in the welding process with the conditions set by the company and conduct briefings before doing work and after work.
