Abstract
Production failure at PT. Taspro is caused by human factors (machine maintenance is not optimal), engine (mechanical problems) and operational problems (cavitation, reduced fluid pressure, rotation does not want to increase, temperature rises). Several production failures were caused by product defects, namely Bearings (wear/shrinkage), Mechanical Seals (leaking), Impellers (corrosion) and Shafts (eroding). Failures that occur can cause a decrease in temperature in each unit by 28-29 degrees Celsius. To reach the temperature of 24°C recommended by building management, an analysis using the FTA (Fault Tree Analysis) and FMEA (Failure Mode and Effect Analysis) methods is required. This method can help identify and detect errors that could cause product malfunction. The results of the FMEA method produce an RPN (Risk Priority Number). The RPN value obtained will be a reference for prioritizing corrective actions. The results of the proposed improvements for the highest cause of failure for the new RPN values are 40, 24, 24 and 15 with a priority level of 4 found in the Bearing component Keywords: FTA (Fault Tree Analysis), FMEA (Failure Mode and Effects Analysis), and RPN (Risk Priority Number) Kegagalan produksi pada PT. Taspro disebabkan oleh faktor manusia (perawatan mesin tidak optimal), mesin (masalah mekanik) dan masalah operasional (kavitasi, tekanan fluida berkurang, putaran tidak mau naik, suhu naik). Beberapa kegagalan produksi disebabkan oleh cacat produk yaitu Bearing (aus/susut), Mechanical Seal (bocor), Impeller (korosi) dan Shaft (terkikis). Kegagalan yang terjadi dapat menyebabkan penurunan suhu di setiap unit sebesar 28-29 derajat Celcius. Untuk mencapai suhu 24°C yang direkomendasikan oleh manajemen gedung, diperlukan analisis menggunakan metode FTA (Fault Tree Analysis) dan FMEA (Failure Mode and Effect Analysis). Metode ini dapat membantu mengidentifikasi dan mendeteksi kesalahan yang dapat menyebabkan kegagalan fungsi produk. Hasil dari metode FMEA menghasilkan RPN (Risk Priority Number). Nilai RPN yang diperoleh akan menjadi acuan untuk memprioritaskan tindakan perbaikan. Hasil usulan perbaikan untuk penyebab kegagalan tertinggi nilai RPN baru adalah 40, 24, 24 dan 15 dengan tingkat prioritas 4 terdapat pada komponen Bearing Kata kunci: FTA (Fault Tree Analysis), FMEA (Failure Mode and Effects Analysis), dan RPN (Risk Priority Number)
