Abstract
PT Future Pipe Industries is a multinational company engaged in the manufacturing industry with its main product being fiberglass pipes. The percentage of Rejection Rate for period January-December 2021 exceeds the set target of 3.09% while PT Future Pipe Industries' target rejection rate is 1.5%. The product defects are dry spots, leaks, cracks, pipe bent, delamination, and dimension out. Problems like this require a quality control so that the high level of defects can be reduced and not exceed the targets that have been set. There are two tools that can be used to assist quality control, namely Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA). FMEA can identify the risk of failure during the production process of making fiberglass pipes. With FMEA, the largest failure mode can be identified through the value of the Risk Priority Number (RPN). Based on the results of the highest failure mode RPN, the FTA method will be used to build a fault tree, to see the root cause of the failure mode. The results showed that the highest RPN value was 343 with dosing error failure mode. Based on the FTA, the root cause of the dosing error problem is influenced by man, material, and machine factors. The proposed improvement for this case is to repair the heater and temperature sensor in the resin and hardener tanks. Keywords: Quality Control, FMEA, FTA PT Future Pipe Industries merupakan perusahaan multinasional yang bergerak dibidang industri manufaktur dengan produk utamanya adalah pipa berbahan fiberglass. Pada saat ini perusahaan mengalami masalah yaitu presentase Rejection Rate periode Januari-Desember 2021 melebihi target yang telah ditetapkan yaitu sebesar 3.09% sementara target rejection rate PT Future Pipe Industries adalah 1.5%. Kecacatan produk tersebut berupa dry spot, leak, crack, pipe bent, delamination, dan dimension out. Masalah seperti ini dibutuhkan adanya sebuah pengendalian kualitas agar tingginya tingkat kecacatan dapat berkurang dan tidak melewati target yang sudah ditetapkan. Ada dua tool yang dapat digunakan untuk membantu pengendalian kualitas yaitu Failure Mode and Effect Analysis (FMEA) dan Fault Tree Analysis (FTA). Penggunaan FMEA dapat mengidentifikasi risiko kegagalan selama proses produksi pembuatan pipa fiberglass. Dengan FMEA dapat diketahui moda kegagalan terbesar melalui nilai Risk Priority Number (RPN). Berdasarkan hasil RPN moda kegagalan yang tertinggi, akan digunakan metode FTA untuk membangun fault tree, untuk melihat akar permasalahan dari moda kegagalan tersebut. Hasil penelitian menunjukkan nilai RPN tertinggi yaitu 343 dengan moda kegagalan dosing error. Berdasarkan FTA maka akar penyebab masalah dosing error dipengaruhi oleh faktor man, material, dan machine. Usulan perbaikan untuk hal tersebut adalah perbaikan pemanas dan sensor suhu pada tanki resin dan hardener. Kata Kunci: Pengendalian Kualitas, FMEA, FTA
