Abstract

This thesis aimed to analyze some factors that cause the error delivery from the warehouse by taking case study at PT. Niro Ceramic Sales Indonesia. This reasearch aimed to identify the factors that cause errors delivery from the warehouse using the Five Whys Analysis and Fishbone Analysis. Results showed that some of the factors that caused the error shipments from the warehouse are : humans include less training and less working hours; methods include SOP demand, no exception rule order for loyal customers, the material covering the time the order is not right, the media include inadequate lighting, indoor conditions not worth it. Recommendations for the management of the company, which is routine training for all warehouse personnel, increasing the number of warehouse personnel, changing working hours to two shifts and evaluate procedures daily expenditures for delivery to customers or for promotion to a dealer or shop. Keywords : Error Delivery, Five Whys Analysis, Fishbone Analysis Tesis ini menganalisa tentang faktor-faktor yang menyebabkan kesalahan pengiriman barang dari gudang dengan mengambil studi kasus di PT. Niro Ceramic Sales Indonesia. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang menyebabkan kesalahan pengiriman barang dari gudang dengan menggunakan metode 5 Whys Analysis dan Fishbone Analysis. Hasil penelitian menunjukkan adanya beberapa faktor yang menyebabkan kesalahan pengiriman barang dari gudang yaitu : manusia meliputi kurangnya pelatihan dan kurangnya jam kerja; metode meliputi SOP permintaan, tdk ada aturan pengecualian order, material meliputi waktu order tidak tepat, media meliputi penerangan tidak memadai, kondisi ruangan tidak layak. Rekomendasi bagi manajemen perusahaan, yaitu mengadakan pelatihan rutin bagi seluruh personil gudang, menambah jumlah personil gudang, merubah jam kerja menjadi 2 shift dan mengevaluasi prosedur pengeluaran barang sehari hari baik untuk pengiriman ke pelanggan ataupun untuk promosi ke dealer atau toko.
