Abstract

A shoe manufacturing MSME in West Jakarta recorded 21 workplace accident cases during the observation period from late 2024 to late 2025, with most incidents occurring in the packing process due to poorly organized work areas and workers’ low compliance with the use of Personal Protective Equipment (PPE). These conditions also indicate that the implementation of the Occupational Health and Safety (OHS) management system in the production line has not yet been optimal. This study aims to identify potential hazards, analyze risk levels, and propose risk control measures in the production process to support the achievement of a zero�accident workplace. The study employed the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method, with risk levels calculated based on the AS/NZS 4360 standard using the formula Risk Level = Likelihood × Severity. Primary data were collected through direct observation in the production area and interviews with the production supervisor, while secondary data were obtained through a literature review. The identified hazards were further analyzed using a fishbone diagram to determine the root causes of the highest-priority risks. The results identified nine hazard variables (X1–X9), consisting of one High-risk variable (11.1%), namely X7, which involved the risk of a worker's hand entering the press machine with a Risk Level of 12, potentially resulting in crush injuries or finger amputation; seven Medium-risk variables (77.8%); and one Low-risk variable (11.1%). The root causes of the highest risk were attributed to human factors, including lack of concentration and non-compliance with occupational safety procedures, as well as machine-related factors, particularly inadequate inspection and routine maintenance. Risk control recommendations were developed based on the hierarchy of controls, prioritizing the installation of safety guards and safety sensors on the press machine to reduce the risk level of X7 from High to Medium or Low. UMKM produksi sepatu di Jakarta Barat mencatat sebanyak 21 kasus kecelakaan kerja selama periode pengamatan akhir tahun 2024 hingga akhir tahun 2025, yang didominasi oleh kecelakaan pada proses packing akibat kondisi area kerja yang tidak tertata serta rendahnya kepatuhan pekerja dalam menggunakan Alat Pelindung Diri (APD). Hal ini juga menjadi sumber permasalahan belum optimalnya penerapan sistem manajemen keselamatan dan kesehatan kerja pada lini produksi. Penelitian ini bertujuan mengidentifikasi potensi bahaya, menganalisis tingkat risiko, dan menyusun usulan pengendalian risiko pada proses produksi guna mendukung tercapainya kondisi zero accident. Metode yang digunakan adalah Hazard Identification, Risk Assessment, and Risk Control (HIRARC), dengan tingkat risiko dihitung berdasarkan standar AS/NZS 4360 melalui rumus Risk Level = Likelihood × Severity. Data primer diperoleh melalui observasi langsung di area produksi dan wawancara dengan supervisor produksi, sedangkan data sekunder diperoleh dari studi literatur; hasil identifikasi bahaya kemudian dianalisis lebih lanjut menggunakan fishbone diagram untuk menelusuri akar penyebab risiko tertinggi. Hasil penelitian mengidentifikasi 9 variabel potensi bahaya (X1–X9), terdiri atas 1 variabel kategori high (11,1%) yaitu X7 berupa risiko tangan masuk ke area mesin press dengan Risk Level 12 yang berpotensi menyebabkan crush injury hingga amputasi jari, 7 variabel kategori Medium (77,8%), dan 1 variabel kategori Low (11,1%). Akar penyebab risiko tertinggi berasal dari faktor manusia berupa rendahnya fokus dan kepatuhan terhadap prosedur K3, serta faktor mesin berupa minimnya pemeriksaan dan pemeliharaan rutin, temuan ini turut konsisten dengan data insiden aktual di lapangan. Usulan pengendalian disusun berdasarkan hierarki pengendalian risiko, dengan prioritas pemasangan safety guard dan sensor pengaman pada mesin press untuk menurunkan Risk Level X7 dari kategori High menjadi Medium atau Low.
