Abstract
for both workers and companies. This study aims to identify potential hazards, assess risk levels, and determine appropriate risk control measures for workplace accidents in the Flexo machine division at PT XYZ. The method employed in this research is Hazard Identification, Risk Assessment, and Determining Control (HIRADC) with a risk matrix approach based on the AS/NZS 4360:2004 standard. Data collection was conducted through field observations, interviews with workers, and review of safety documents available within the company. The findings are expected to classify risk levels into four categories, namely low, moderate, high, and extreme, in accordance with the parameters established by the aforementioned standard. Furthermore, this study formulates risk control recommendations based on the hierarchy of controls, encompassing elimination, substitution, engineering controls, administrative controls, and the use of personal protective equipment. The results of this research are expected to serve as a strategic reference for the company in designing a comprehensive occupational health and safety program, thereby reducing workplace accident rates and enhancing operational safety on a sustainable basis. Keywords: HIRADC, Hazard Identification, Risk Assessment, Hierarchy of Controls, Occupational Health and Safety Kecelakaan kerja merupakan permasalahan serius yang dapat menimbulkan kerugian bagi tenaga kerja maupun perusahaan. Penelitian ini bertujuan untuk mengidentifikasi potensi bahaya, menilai tingkat risiko, serta menentukan langkah pengendalian risiko kecelakaan kerja pada divisi mesin Flexo di PT XYZ. Metode yang digunakan dalam penelitian ini adalah Hazard Identification, Risk Assessment, and Determining Control (HIRADC) dengan pendekatan matriks risiko berdasarkan standar AS/NZS 4360:2004. Pengumpulan data dilakukan melalui observasi lapangan, wawancara dengan tenaga kerja, serta penelaahan dokumen keselamatan yang tersedia di perusahaan. Hasil penelitian diharapkan mampu mengklasifikasikan tingkat risiko ke dalam empat kategori, yakni rendah, sedang, tinggi, dan ekstrem, sesuai dengan parameter yang ditetapkan dalam standar tersebut. Selain itu, penelitian ini juga merumuskan rekomendasi pengendalian risiko berdasarkan hierarki pengendalian yang meliputi eliminasi, substitusi, rekayasa teknik, pengendalian administratif, serta penggunaan alat pelindung diri. Hasil temuan dalam penelitian ini diharapkan dapat menjadi acuan strategis bagi perusahaan dalam merancang program keselamatan dan kesehatan kerja yang komprehensif, sehingga mampu menekan angka kecelakaan kerja dan meningkatkan keselamatan operasional secara berkelanjutan.
